Established by Medicare, AltaMed PACE offers a suite of services including care coordination, transportation, social services, and meals within a nurturing environment. The program emphasizes dignity, respect, and compassion, fostering community connections and enhancing the quality of life for participants. Since its inception in 1996, AltaMed PACE has expanded to 15 centers, serving over 4,700 seniors annually, and continues to be a pivotal element of AltaMed's success.
